1) What are the steps required to register for an account and a health screening appointment?

Step 1: For a first-time user, enter your NRIC/FIN/Passport number and click on “First-Time User of this Portal”. For returning user of the portal, enter your NRIC/FIN/Passport number and click on “Returning User – Login Now”.

 

Step 2: Fill in your particulars in the form. Fields with an asterisk are required to be filled in. Ensure that the checkbox is marked and click on “next” to proceed.

 

Step 3: You will be shown a summary of your registration profile. Check to ensure that the information entered is correct, if not, click on “amend particulars” to change the information or click on “next” to proceed.

 

Step 4: If the screen below is shown, this means your registration has been successful. Click on the button to proceed.

 

Step 5: Select your preferred date/location and time by clicking on the down arrow and confirm by clicking “next”.

 

Step 6: Select your preferred package and optional tests and click “next” to proceed.

 

Step 7: You will be required to complete the survey before moving on to the next step.

 

Step 8: You will be shown a summary of your health screening registration process. check to ensure that the information chosen is correct, if not, you may change the information by clicking on the “edit” buttons or click “confirm” to proceed.

 

Step 9: Your health screening registration is complete.

 


2) Why am I unable to login?

It could be due to the following reasons:

-Your password is incorrect. Please check and ensure if your capslock is off or on. If you have forgetten your password, you can request for a new password to be sent to your email, using the ‘Forgot your password’ in the member’s login page.

-Your account is locked. You might have entered the wrong password for more than 3 times. You will be required to click here and provide us with your name, NRIC, email and mobile no. to request for the unlock of your account.

 


3) What is CIC?

CIC, which stands for Company Identification Code, is a unique code created for your organisation’s health screening registration. It is used to direct you to the relevant health screening information such as dates and packages. It will be embedded in your registration url through the email sent by your health screening coordinator.

 


4)Where do I locate the CIC?

Should you need be prompted to key in your CIC, please refer to your health screening coordinator’s email. It can also be found on the embedded registration link as follows:

 


5) How do I register for my family members?

You will be required to create a new account for each participating family member. You cannot reuse your account for multiple registration. This is to facilitate future features such as accessing your health screening report online or managing your personal medical records.

 


6) How do I change/cancel my health screening appointment? Or register a new appointment?

After logging in to your account, you will find this link within the drop menu located in the top, titled ‘Health Screening‘.

To change or cancel your health screening appointment, click on “View/Change of Apppointment“. To register a new appointment, click on “Health Screening Registration“. Do note that you can only register for 1 appointment at any time until you have attended it.

 


7) How do I complete the additional lifestyle survey?

After logging in to your account, go to “View/Change of Appointment” which is located on the top within ‘Health Screening’. Click on it and the screen will be displayed as below.

Click on “Link” under Lifestyle Survey to get access to the Lifestyle Survey. When you have completed and submitted the survey, please ensure the screen is fully loaded to confirm your submission.

 


8) How do I print the Confirmation Letter for my appointment?

After logging in to your account,  go to “View/Change of Appointment” which is located on the top within ‘Health Screening’. Click on it and the screen will be displayed as below.

Click on “Print Confirmation Letter“. A new window will appear and you may print it’s content.
